A connected patient ecosystem in long-term care: what a shared experience looks like in practice

Copenhagen  —

July 20, 2026

Patients and caregivers managing long-term treatment often spend far more time at home than they do in the clinic. While therapies may be well established, maintaining treatment routines over months and years can be challenging. Healthcare professionals need visibility into what happens between appointments, while patients and caregivers need support that fits into everyday life.

Why connection matters

One of the underlying problems is that everyone involved often has only part of the picture. Patients and caregivers manage treatment at home, healthcare professionals have limited visibility into what happens between appointments, and important information is often fragmented across different tools and systems.

The result is a disconnected experience that makes it harder to provide timely support when it is needed most. The goal is not simply to digitalize the treatment journey - it is to connect it.

Building a shared experience

Dawn Health developed an ecosystem of integrated web and mobile solutions designed to support patients, caregivers, and healthcare professionals through a shared experience.

Patients gained access to treatment reminders, injection tracking, symptom tracking, and educational resources. Healthcare professionals could follow adherence and progress over time, enabling more informed conversations and earlier intervention when needed. The solution was built from the outset to support global deployment across markets, languages, and regulatory environments.

What changed

Research in this therapeutic area has consistently shown that connected digital support can help improve adherence and treatment outcomes for patients receiving growth hormone therapy. ¹ ²

Today, the Dawn Health ecosystem supports more than 25,000 patients and caregivers and more than 3,500 healthcare professionals across 40 markets and over 30 languages.

What makes this case particularly interesting is that the impact was not driven by a single feature or intervention. It came from addressing the practical challenges that patients and caregivers face every day, including complexity, lack of support, and limited connection between life at home and care in the clinic.

For pharmaceutical companies, it is a reminder that digital health creates the most value when it becomes part of the treatment experience rather than sitting alongside it. Technology plays an important role, but the real difference comes from connecting patients, caregivers, and healthcare professionals around a shared treatment experience.
