Rare glomerular diseases companion app

Glomerular diseases

Supporting patients with rare glomerular diseases through symptom tracking, emotional well-being support, and structured care coordination

Developed in collaboration with Novartis Pharma AG, Nelia is a digital companion designed for patients living with rare glomerular diseases.   The app empowers users to better understand their condition, support their mental health, and stay on top of treatment-related tasks.

Challenge

Living with rare glomerular diseases can feel overwhelming. Many young patients transition from receiving parental care to self-management while dealing with daily symptoms, complex medication regimensregimes, and emotional challenges. At the same time, healthcare providers often lack structured, day-to-day insights that can help guide and personalize treatment.

Approach

We worked closely with Novartis and a broad network of partners to develop Nelia. Through stakeholder interviews and iterative development, we identified patient needs across physical and emotional domains.

Built on the Dawn Platform, the app combines components from our product suite with customized features to deliver a highly personalized experience. Nelia consolidates health insights and emotional support into one intuitive solution.

Solution

Nelia helps patients to track their physical symptoms and treatment routines while also supporting their emotional well-being. The app enables users to log data over time such as fatigue, edema, proteinuria, blood pressure, and body weight over time  to better understand their condition.

Beyond data, Nelia has been developed to support emotional resilience and help patients manage the unpredictability of their condition. With a content library of 41 pieces, users get a personalized experience throughout the app. The library includes articles spanning daily life management and mental health support, to practical exercises designed to help patients cope with the emotional impact of their condition.

The app is designed to improve care coordination by structuring health information in a way that’s easy to share with HCPs.
